Immunizations
Prior to
starting school state law requires that parents provide proof of immunizations
for certain diseases. See below for a list of age specific immunizations.
Tennessee School Immunization Certificates must be completed and signed by your
medical provider prior to enrolling your child in Kindergarten. Contact the
