ENROLLMENT INFORMATION
Student
Enrollment Information
Parents: Remember to notify the school of address
change, changes in contact information, or medical or allergy updates.

New Student Enrollment Information
The following items
are required for enrollment in the South Carroll County Special School
District:
Proof
of residence in the district. (Examples: rent receipt or utility bill in the
parent/guardian’s name giving the 911 street address of residence.)
Name,
address and phone number of previous school. Student must be in good standing
(all books returned, fees paid) at the previous school before records are
released.
Present
student birth certificate (Not Mother's Copy), and Tennessee Health
Record including proof of immunizations and physical. City, county, state and
country of birth are required as well as mothers maiden name. Out-of-state
transfer students should visit the Carroll County Health Department to transfer
records to proper Tennessee documentation. All non-Tennessee transfer students
must present proof of physical exam from a Tennessee physician and immunizations
on Tennessee’s approved form.
Complete
student enrollment data form and medical release form. These forms may be
completed in the school counselor's office. You may download the enrollment form and medical release (Adobe Reader required) and bring completed forms to
the counselor’s office to enroll.
Complete
Home Language Survey. The Language Survey is available from the school
counselor.
High
school transfer students should bring a copy of their transcript of completed
and current coursework. Clarksburg High School is on the four block, semester
class schedule. Students transferring from schools with six/seven period day
schedules can expect to lose credits and/or participate in credit recovery after
school.
